仰付御知行百石無御相違忰伝右衛門ニ
被下置私儀ハ是迄之通相勤候様被

月番ともニ御免被成之旨被 仰出

病死法名帰壽院了山知足居士葬所

現代語訳
師として再び仰せ付けられた。
一 同十四年巳年九月、御擬作を差し上げたい旨を願い出て許可された。
一 宝永五年子年三月、御知行百石を下し置かれ、前々の通り御側医師として再び仰せ付けられた。
一 正徳元年卯年十二月、願いの通り隠居を仰せ付けられ、御知行百石は相違なく息子の伝右衛門に下し置かれ、私（松意）は今まで通り勤めるよう仰せ付けられた。
一 同三年巳年二月、願いの通り御城御番並びに月番ともに御免になる旨が仰せ出された。
一 享保十四年酉年六月朔日、七十八歳で病死した。法名は帰壽院了山知足居士、葬所は萬松山天寧寺。

he was reappointed as a physician.
一 In the 9th month of the 14th year, Year of the Snake, he petitioned to submit a treatise on medical practice, which was granted.
一 In the 3rd month of Hōei 5, Year of the Rat, he was granted a fief of 100 koku and was reappointed as personal physician as before.
一 In the 12th month of Shōtoku 1, Year of the Rabbit, as requested, he was granted permission to retire. His fief of 100 koku was transferred without change to his son Denemon, and he (Matsui) was ordered to continue serving as before.
一 In the 2nd month of the 3rd year, Year of the Snake, as requested, he was exempted from both castle guard duty and monthly rotation duty.
一 On the 1st day of the 6th month of Kyōhō 14, Year of the Rooster, he died of illness at the age of 78. His posthumous Buddhist name was Kijuin Ryōzan Chisoku Koji, and he was buried at Banshōzan Tenneiji Temple.
