一 宝永元申年正月願之通永代之御
暇被下置他邦ニ罷在候処親被
召出候ニ付又々願之上罷帰帰嫡被

百石無御相違被下置小番被 仰付候
但親之家業不続者ハ跡式被
仰付様可在之旨先達而被

堀田相模守様の御家来
妻 平野甚右衛門某の娘
一 宝永元年申年正月、願いの通り永代の御暇を下し置かれ他国に滞在していたところ、親（松意）が召し出されたことにより、再び願い出て帰国し、家督相続を仰せ付けられた。
一 正徳元年卯年十二月、家督として御知行百石を相違なく下し置かれ、小番を仰せ付けられた。
ただし、親の家業を継がない者には跡式を仰せ付けるべき旨が先達て仰せ出されているが、先年御暇を申し請けて他国へ遣わし、近頃手前へ引き取ったことであるから、家業伝授をする間もない。その上、知足（重次）は内外の療治をよく行うので、格別の御情により仰せ付ける旨が仰せ出された。
一 同三年巳年三月、勤番として江戸表へ上り、同九月に下った。
一 同五年未年十二月、御供番を仰せ付けられた。

一 In the 1st month of Hōei 1, Year of the Monkey, as requested, he was granted permanent leave and was residing in another domain. However, when his father (Matsui) was summoned back to service, he petitioned again to return and was ordered to succeed to the family headship.
一 In the 12th month of Shōtoku 1, Year of the Rabbit, he was granted the family inheritance of 100 koku without change and was appointed to the kobangumi (junior guard unit).
Although it had been previously announced that those who do not continue their father's profession should be given other positions, since he had been granted leave and sent to another domain in previous years and was only recently brought back, there was no time for him to receive instruction in the family profession. Moreover, since Chisoku (Shigetsugu) performed medical treatment both internally and externally well, this appointment was made as a special favor.
一 In the 3rd month of the 3rd year, Year of the Snake, he went up to Edo for guard duty rotation and returned in the 9th month of the same year.
一 In the 12th month of the 5th year, Year of the Sheep, he was appointed to the gokōban (lord's escort guard).
