一、本宮へお供えした幣物四合を御文庫へ納められた。
一、神主博久と片岡祢宜功久から宣命・宣詔等が提出された件
その控えは以下の通り
天皇の詔として、畏れ多き賀茂皇大神の御前に、恐れ多くも申し上げます。神社が次第に損傷したため、新たに修造を加え、正殿を造り替え、輪奐として美しく成し、今遷御を催し給う。風雨も無難に、無事無故に、御意の随意に遂げ給わんことは、皇大神の深き御恵み、広き御助けによるものと思い奉る故に、ここに吉日良辰を選び定めて、正二位権大納言藤原朝臣隆望、弾正少弼正五位下源朝臣惟栄
等を差し遣わし、礼代の大幣を捧げ持たしめて奉り出し給う。畏れ多き大神よ、この状を平らかに安らかに聞こし召して、天皇の朝廷を宝位動くことなく、常盤堅盤に夜守り日守りに護り幸い給い、一天泰平に、四海静謐に護り恤み給えと、恐れ多くも申し給う。

The record is as follows:
By imperial decree, we reverently address the august Kamo Imperial Deity: As the shrine had gradually fallen into disrepair, we have undertaken new repairs and reconstruction, rebuilt the main hall in magnificent splendor, and now celebrate the ritual relocation. That this may be accomplished without difficulty from wind and rain, safely and without incident, according to the imperial will, we believe depends upon the deep benevolence and broad assistance of the Imperial Deity. Therefore, selecting and determining an auspicious day and favorable time, we dispatch Senior Second Rank Acting Major Counselor Fujiwara no Ason Takamochi, Junior Assistant in the Imperial Prosecuting Office Junior Fifth Rank Lower Source no Ason Koreyoshi,
and others, having them bear the great ritual offerings in place of the ceremony and present them. August Deity, hear this petition peacefully and tranquilly, protect the Emperor's court so that the throne remains unmoved, firm as eternal rock, guarding night and day, bringing prosperity, keeping all under heaven in great peace and the four seas in tranquil calm, protecting and caring for all—this we reverently petition.
