現代語訳
地蔵菩薩と鬼とが互いに争い、二つの頭を連れて地獄と極楽の境までやって来た。地蔵菩薩は薬の金縛りを取り出して、善の頭の分の右腕に白い薬を塗り与えた。鬼は手早く釘抜きを取り出して、悪の首の舌を抜こうとする。このように争ったけれども、身体は一つなのでどうしようもなく、閻魔王と地蔵菩薩とが相談の上、「まず今回は許して現世へ帰し、この上もし悪の首がいよいよ罪を作るなら、善の首とともに地獄へ落とすべし。もしまた善の首がますます善言をなすなら、悪の首の旧悪を許し、ともに極楽へ導くべし」として現世へ帰された。こうして夢は覚めた。
すべての人に善をなしても、大きな悪のためにその功徳を失う。朝に罪を作っても、夕べに改めれば、すなわち善人となる。悪よりも善に移ろうと思えば移ることができる。早く悪を捨てて善に進むべしと、地蔵菩薩が懇ろにお示しになった。

「見る目が美しい顔立ちなら、養子に欲しいと言うであろう」

「お前の舌を抜いて墨染めにしてやろう」

「息を詰めて嫌がっても、薬が散ってはならぬぞ」

英語訳
Jizo Bodhisattva and the demon fought with each other and came to the boundary between hell and paradise, pulling the two heads along. Jizo Bodhisattva took out medicinal paralysis powder and applied white medicine to the right arm belonging to the good head. The demon quickly took out nail pullers and tried to pull out the tongue of the evil head. Though they fought like this, since there was only one body, there was nothing to be done. King Enma and Jizo Bodhisattva consulted and decided: "For now, we shall forgive and return him to the mortal world. If the evil head continues to commit more sins, both the good head and evil head shall be cast into hell. But if the good head practices even more good words, we shall forgive the evil head's past sins and guide them both to paradise." Thus he was returned to the mortal world, and the dream ended.
Even if one does good to all people, great evil causes one to lose that merit. Even if one commits sins in the morning, if one repents in the evening, one immediately becomes a good person. If one wishes to move from evil to good, it is possible. One should quickly abandon evil and advance toward good - this is what Jizo Bodhisattva earnestly taught.
