現代語訳
恐れながら申し上げます。
私は思いがけず商札目付を仰せつけられ、重ね重ねありがたい仕合わせと存じ、勤めておりましたが、先頃から病気になり、だんだんと重い病気となって勤めることが困難と存じます。よって、お赦免をいただきますよう願い上げます。

Respectfully submitted,
I unexpectedly received the appointment as commercial license inspector, which I considered a most grateful honor, and have been diligently serving in this capacity. However, I recently fell ill and my condition has gradually worsened to the point where I find it difficult to continue my duties. Therefore, I humbly request to be relieved of this position.
