是心を尽寄特なる成【衍ヵ】儀ニ付為御褒
美金弐百疋被下置候事

之治与申者深手負候ニ付罷越療
治いたし候様被 仰付罷越療
治之上全快 仕候事
一 同年八月軍中之療治相心得候
由ニ付御立場被相詰本道外科
之医師ニ加り 尤其節ハ帯刀ニ而
罷出様被 仰付罷出候事

御勘定所江相納候様被 仰付候

現代語訳
【右丁】
これは心を尽くした特別な働きであることから、御褒美として
金二百疋を下し置かれた件
一 同三年亥年六月、小田付村において巳之治という者が深手を負ったことにより、出向いて治療
を行うよう仰せ付けられ、出向いて治療
の上、全快させた件
一 同年八月、軍中の治療を心得ているとのことにより、御立場に詰めることとなり、本道外科
の医師に加えられた。もっとも、その節は帯刀にて
出仕するよう仰せ付けられ、出仕した件

一 同年九月、伝来の製薬を御試験のため製造し、
御勘定所へ相納めるよう仰せ付けられた

一 文化元年子年五月
欽文様の御疱瘡前の御洗薬を差し上げる
