乍恐以口上書奉願上候御事
私儀不調法者に御座候処不存寄組頭役
奉蒙 仰難有仕合奉存相勤罷在候処去冬
中ゟ病気之処追々大病罷成組頭御役
難相勤奉存候依之御赦免被成下置候様奉願上候

現代語訳
恐れながら口上書をもって願い上げ申し上げます
私は不調法者でございますが、思いがけず組頭役を仰せつかり、ありがたい仕合わせと存じ相勤めておりましたところ、去年の冬頃から病気となり、だんだんと大病となってしまい、組頭の御役を勤めることが難しくなったと存じます。つきましては御赦免くださいますよう願い上げます。

右の願書を十日に御役所において坂井氏が御取次として差し上げましたところ、即日御聞き置きいただき、御沙汰の後、役の入札を先例の通り仰せつけられ、十二日に申し上げるよう御沙汰があったので、組頭弥右衛門を呼んで申し渡しました。

Humbly submitted as a written petition
I am an inadequate person, but unexpectedly received the appointment of group leader (kumigashira), which I considered a grateful fortune and have been diligently serving. However, since last winter I have been ill, and it has gradually become a serious illness, making it difficult for me to continue serving in the role of group leader. Therefore, I humbly request to be granted release from this duty.

The above petition was submitted on the 10th day through Mr. Sakai as intermediary at the government office. It was immediately acknowledged, and after deliberation, it was ordered that the position be filled by lottery according to precedent. Notice was given to report on the 12th day, so group leader Yaemon was called and informed.
