先祖代々下野国宇都宮ニ居住仕候田代右近ト申者古河
公方エ相勤申候晴氏公墨印二通于今所持仕候天正年中
宇都宮下野守没落之砌右近儀江戸エ罷出剃髪仕医師ニ
罷成田代養元綱通ト申候其頃大久保相模守殿土井大炊
頭殿 権現様御前ニテ吹挙有之医業モ能其上関東ニテ
武功モ有之者之由被達 御聴候へハ早速御医師並被
召出拝知三百石被下其後大坂御陣之節 台徳院様ノ御
眼前ニテ高名仕其後駿河大納言様エ御附人ニ罷成祝髪

当代田代養元道存《本国下総 生国越前｜姓不明》
先祖代々下野国宇都宮に居住していた田代右近という者が古河公方に仕えていた。晴氏公の墨印二通を今でも所持している。天正年中（1573-1592年）、宇都宮下野守が没落した際、右近は江戸に出て剃髪し、医師となって田代養元綱通と名乗った。その頃、大久保相模守殿と土井大炊頭殿が権現様（徳川家康）の御前で推挙し、「医業も優れており、その上関東で武功もあった者である」と申し上げたところ、すぐに御医師並みに召し出され、拝領知三百石を下された。その後大坂の陣の際、台徳院様（徳川秀忠）の御前で高名を立て、その後駿河大納言様（徳川忠長）に御附人として仕え、祝髪を命じられて田代大膳と名乗った。その後御加増二百石を下され、御先物頭役を勤め、御上洛の時分には御先乗りを務めるなど段々と勤めていたが、駿河様が御落去の後、御家士はいずれも諸国に御預け物として仰せ付けられた。その節

An ancestor named Tashiro Ukon, who had resided in Utsunomiya, Shimotsuke Province for generations, served the Koga Kubō. He still possesses two ink seals from Lord Hareuji. During the Tenshō period (1573-1592), when Utsunomiya Shimotsuke-no-kami fell from power, Ukon went to Edo, took the tonsure, became a physician, and called himself Tashiro Yōgen Tsunamichi. At that time, Lord Ōkubo Sagami-no-kami and Lord Doi Ōi-no-kami recommended him before Gongen-sama (Tokugawa Ieyasu), saying "He is skilled in medicine and moreover has military achievements in the Kantō region." He was immediately summoned as a court physician and granted a stipend of 300 koku. Later, during the Osaka campaigns, he distinguished himself before Taitorku-in-sama (Tokugawa Hidetada). Subsequently, he served as an attendant to Suruga Dainagon-sama (Tokugawa Tadanaga), was ordered to take the tonsure, and was called Tashiro Daizen. Later he received an additional stipend of 200 koku, served as captain of the advance guard, and performed various duties including serving as an advance rider during imperial visits to Kyoto.
