いふとも種痘（しゆとう）の醫者（いしや）ごとに名醫（めいい）にもあらず晝（ひる）は死（しぬ）る病（やまひ）の起（おこる）を朝（あさ）はし

というとも、種痘の医者がことごとく名医であるわけではなく、昼には死ぬ病気が起こることを朝には知らない者の多い世の中なので、病気があるかないかを見分けることも難しいことである。特に平生病気がちで危険に見える子供も痘瘡を軽くする者もあれば、必ず病気があるゆえに重くするとも決めがたい。およそ人の身に病気が起こるのは物に黴が付くのと同じで、物を湿地に置けば黴び、乾燥した所にあれば黴びない。病気もこれと変わることはなく、たとえ病むべき身であっても病むべき原因を遠ざければ病むことはなく、病むべき身だからといって強いて病ませるのは、黴びるものだからといって湿地に置くのと同じことで、その害は目前である。かかる煩わしい業をするよりも、一向にこの病気を患うことがなければ、どれほどめでたいであろうか。痘瘡は気運・時令の疫癘ではなく、元来

日本の土地の気によって起こるものではない。梅毒や疥癬と変わることのない伝染病であるのは、今見るとおり避けて病まない土地が数多あるのを見て知るべきである。古くは代々の朝廷より遣唐留学の使として外国へ行かれる公卿も数多あり、また帰化として外国より来住する者も多かった。それのみならず乱れた世には私的に往来する者もいたゆえに、おのずから土地にあるべからざる病気も伝わったのである。元より土地にその毒気があって起こる病気は、その気の中に住む人なればいかように身持ちよくても必ず免れるとは言いがたい。身持ちのよい人が自然と病むのは天命である。痘瘡や麻疹のような伝染病で死ぬ者はことごとく非命である。寝起き・飲食・娯楽・苦労が分に過ぎて病死するさえ聖人は非命であるとおっしゃっている。まして病まずして済む病気で死ぬのは非命の甚だしいものである。

Even so, not all variolation doctors are necessarily skilled physicians, and in this world where many do not know in the morning that a fatal disease will arise by noon, it is difficult to distinguish between the presence and absence of disease. Particularly, even children who appear chronically ill and dangerous in daily life may experience smallpox lightly, while it is hard to determine whether it will be severe necessarily due to existing illness. Generally, the arising of disease in the human body is like mold growing on objects - if you place things in damp ground they become moldy, but if in dry places they do not mold. Disease is no different from this: even if one has a body prone to illness, if one distances oneself from the causes of illness, one will not fall ill. To force illness upon a body prone to disease is the same as placing something that molds in damp ground, and the harm is immediately apparent. Rather than engaging in such troublesome practices, how wonderful it would be if one never contracted this disease at all. it is not something that arises from the climate of Japanese soil. That it is a contagious disease no different from syphilis or scabies can be understood by observing that there are now many lands that avoid it and do not suffer from it. In ancient times, there were many court nobles who went abroad as envoys for studying in Tang China sent by successive imperial courts, and there were also many who came to reside here from foreign countries as naturalized citizens. Moreover, in turbulent times there were those who traveled privately, and thus diseases that should not exist in the land were naturally transmitted. Originally, diseases that arise from toxic qi present in the land - since people live within that qi, no matter how well they conduct themselves, it is difficult to say they will necessarily escape it. When people of good conduct naturally fall ill, this is heavenly fate. All those who die from contagious diseases like smallpox and measles meet unnatural deaths. Even when people die from illness due to excess in sleeping, eating, pleasure, or hardship, the sages say this is an unnatural death. All the more so, to die from a disease that could be avoided is the most extreme form of unnatural death.
