なりと思えども全くそうではない。痘瘡は穢気不浄を甚だしく好むゆえに悪症を起こすのである。これは世人の思うこととは正反対の違いである。その理屈はどうかと言えば、元来痘瘡の毒気は人の呼吸に連れて口鼻より入って五臓六腑に通り、一身の経絡に染み渡り、すでに正気を傷つけようとする。正気はその毒気を外へ追い出そうとする。この時正気と毒気とが挑み戦うゆえに、一身の気血が悩乱して忽ち熱気を発する。これを初熱という。
