候事
一 仰せ付け寛保元酉年九月親 御上京
御供被 仰付候ニ付願之上一同罷
登御道中御在京中重軽ともニ
病用相勤候事
一 延享ニ丑年三月若森登りニ付
御附被成被差登候
但右ニ付御内々為御褒美金三
百疋被下置此後右御用度ニ被
仰付毎度同様被下置候事
一 寛延元辰年八月跡式御知行
百石無御相違被下置医師被
仰付是迄被下置候稽古料ハ

現代語訳
その事
一、寛保元年酉年九月、親が御上京の御供を仰せ付けられたので、願い出の上一同で上洛し、御道中・御在京中を通じて重軽ともに病気の治療に相勤めた事
一、延享二年丑年三月、若森登りに付き、御附として差し登らせられた
ただし右に付き、御内々で御褒美として金三百疋を下し置かれ、この後右の御用の度に仰せ付けられ、毎度同様に下し置かれた事
一、寛延元年辰年八月、跡式の御知行百石を相違なく下し置かれ、医師を仰せ付けられ、これまで下し置かれていた稽古料は召し上げられた事
一、宝暦元年未年正月、杏庵と改名したい旨を願い出て、その通りに任せられた事
一、同年二月、御国目付衆の御用を仰せ付けられ、在留中はもちろん、江戸往来の節には附添として仰せ付けられ相勤めた事

That matter
1. In the 9th month of Kanpō 1 (1741), Year of the Rooster, when his father was appointed to accompany the lord's journey to Kyoto, he petitioned and went up to the capital together with the party, serving in medical treatment for both serious and minor ailments during the journey and stay in Kyoto.
1. In the 3rd month of Enkyō 2 (1745), Year of the Ox, for the Wakamori ascent, he was appointed as an attendant and sent up the mountain.
However, for this service, he was secretly given a reward of 300 hiki of gold, and thereafter whenever appointed to such duties, he was given the same amount each time.
1. In the 8th month of Kan'en 1 (1748), Year of the Dragon, he was granted a hereditary stipend of 100 koku without change and was appointed as a physician. The training allowance that had been granted until then was withdrawn.
1. In the 1st month of Hōreki 1 (1751), Year of the Sheep, he petitioned to change his name to Kyōan, and this was approved as requested.
1. In the 2nd month of the same year, he was appointed to serve the domain inspectors (kuni-metsuke), and was ordered to serve not only during their residence but also as an attendant during their travels to and from Edo.
