現代語訳
改めのため罷り越した御代官大野佐左衛門殿の手代沢田忠助と申す者が病気になったので、御人指しをもって仰せ付けられ、早速罷り越し相勤めた事
一、同三年戌年三月、山形へ勤番のため罷り越し、同九月に罷り帰った事
一、同四年亥年三月、勤番のため江戸表へ罷り登った

光勝院様の御用のため御人指しを
もって差し登らせられたので、高木小左衛門へ承り合わせるよう御用人をもって仰せ聞かされ罷り登ったところ、御目見えを仰せ付けられ御熨斗を頂戴し、御次において御吸物・御酒を下され、御扇子一対・御鼻紙・御手拭を下し置かれ、御診を仰せ付けられた事

Because Sawada Chūsuke, an assistant to Magistrate Ōno Sazaemon who had gone to conduct an inspection, fell ill, he was ordered through official designation to go there immediately and served in that capacity.
1. In the 3rd month of the 3rd year (1765), Year of the Dog, he went to Yamagata for guard duty and returned in the 9th month of the same year.
1. In the 3rd month of the 4th year (1766), Year of the Boar, he went up to Edo for guard duty.
However,
because he was sent up through official designation for
the service of Lord Kōshōin, he was told through a retainer to consult with Takagi Kozaemon. When he went up, he was granted an audience, received ceremonial gifts (noshi), and in the reception room was served soup and sake, given a pair of fans, tissue paper, and hand towels, and was appointed as physician.
