風寒を冒（をか）さぬやうに密室（みつしつ）に居て薬は良医に托（たく）すへし飲食（いんしよく）
を慎（つゝしま）ず風寒に再感（さいかん）すれは水気と熱と上（かみ）心胸（しんけう）へ迫（せまつ）て心（しん）を衝（つう）
て死するなり脚気（かつけ）の衝心（しやうしん）は起居（ききよ）も飲食（いんしよく）も元気（けんき）も平生（へいせい）体（てい）
にて物をいひなから心を衝（つ）くなり実（じつ）に恐（おそ）るべきものなり

【右丁】
風寒にかからないように密室にいて、薬は良医に託すべきである。飲食を慎まず風寒に再び感染すれば、水気と熱とが上って心胸へ迫り、心を衝いて死するのである。脚気の衝心は起居も飲食も元気も平常通りで、物を言いながら心を衝くのである。実に恐るべきものである。
心を衝く勢いになっては薬ではどうにもできず、衝心しないように心を用いるべきである。脚気を病まないように普段から慎むべきである。
少年で死ぬべきではないけれども、冷たい物を食べ冷水を飲み風寒にかかって養生が行き届かないために死するのは天命ではない。非命に死すというべきである。霍乱とは吐こうと思っても吐けず、下そうと思っても

下らず腹痛して混乱するのを乾霍乱という。また吐き下しして混乱するのを湿霍乱という。霍乱とはその煩悶の形が鶴の翅を振るうようで、陵乱とは崩れ乱れた様子である。容子をもって名づけたものである。食滞に外邪を受けるものである。飲食を慎まないことから起こる。ただ夏のみならず四季ともにある病気である。

One should stay in a closed room to avoid exposure to wind and cold, and entrust medicine to a good physician. If one does not watch one's diet and becomes infected again with wind and cold, water qi and heat will rise up and press upon the heart and chest, striking the heart and causing death. In beriberi's attack on the heart (shōshin), one's daily activities, eating and drinking, and vitality all remain normal, yet the heart is struck while one is speaking. This is truly frightening.
Once the force that strikes the heart develops, medicine can do nothing, so one must take care to prevent the heart from being attacked. One should be cautious in daily life to avoid contracting beriberi.
Though young people should not die, when they die because their health regimen is inadequate after eating cold foods, drinking cold water, and being affected by wind and cold, this is not natural death but should be called unnatural death. Cholera (kakuran) refers to wanting to vomit but being unable to vomit, wanting to have diarrhea but

being unable to defecate, experiencing abdominal pain and confusion - this is called dry cholera. Also, vomiting and having diarrhea while being confused is called wet cholera. "Kakuran" refers to the form of distress being like a crane flapping its wings, and "ryōran" means a collapsed and disordered state. It was named based on the appearance. It occurs when external pathogens affect food stagnation. It arises from not being careful about food and drink. It is a disease that occurs not only in summer but in all four seasons.
