一 十月十三日先達而切支丹類族御医師田代養山《割書：道｜修》致病死候ニ付
切支丹奉行月番前田安芸守《割書：直｜勝》殿江三寺久左衛門を以左之通
被指出之
越前国足羽郡福居城下転切支丹田代養仙
嫡男自分手医師本人同前養山儀当丑十月
三日八十五歳ニ而致病死候依之即刻検使指遣之
死骸相改候処別条依無御座塩詰ニ申付置候

ただし右の趣旨について御国許において十一月二十八日、町在へ町奉行・
郡奉行が賜った
一 十月十三日、先達てキリシタン類族の御医師田代養山《道修》が病死したため、
キリシタン奉行月番の前田安芸守《直勝》殿へ三寺久左衛門を通じて以下の通り
提出された
越前国足羽郡福井城下転びキリシタン田代養仙の
嫡男、自分手医師、本人同前の養山の儀、当丑十月
三日八十五歳で病死いたしました。これにより即刻検使を派遣し、
死骸を改めたところ別条ございませんので塩漬けにして置いております。
御指図次第申し付けます。以上

Tashiro Yōzan, eldest son of the converted Christian Tashiro Yōsen of Fukui castle town, Asuwa District, Echizen Province,
personal physician, same status as his father, died of illness
at age 85 on the 3rd day of the 10th month of this year of the Ox. Accordingly, an inspector was immediately dispatched,
and upon examining the corpse, no abnormalities were found, so it has been preserved in salt.
