白川（しらかは）法皇（はうわう）は熊野（くまの）御幸（みゆき）あつて。御華（をんはな）参（まゐ）らせたまふ
折（をり）からにて。触穢（しよくゑ）出（いて）きにけりと。急（いそ）き御輿（みこし）にめされ
辛（から）うして都（みやこ）六条殿（ろくてうとの）に還幸（くはんこう）なり。南庭（なんてい）に仮（かりや）を設（まう）けて
御座（ござ）とし給ふ主上（しゆせう）は宝輿（ほうれん）に御（めし）て池（いけ）の汀（みきは）に御幸（みゆき）なる

鶴ヶ岡八幡宮（つるがおかはちまんぐう）震動（しんどう）。七月廿三日大地震。夫（それ）より七十四歳
を経（へ）て。同九十一代 伏水（ふしみ）帝 永仁（ゑいにん）元年 癸巳（みつのとのみ）歳四月廿三日

現代語訳
雷のような音であった。立ち昇る塵と埃は黒い煙のようで、天を覆って日の光を見ることができなかった。山が崩れて川を埋め、海に流れ込んで浜辺を変えた。大地は稲妻のように裂けて水が湧き出し、巨石が割れて谷に転がり落ちた。人民と家畜の死者は数知れなかった。このとき白河法皇は熊野御幸の最中で、御精進潔斎をしておられる折からのことで、穢れに触れてしまったということで、急いで御輿に乗られ、かろうじて都の六条殿にお帰りになった。南庭に仮の建物を設けて御座所とし、主上（天皇）は宝蓮（輿）にお乗りになって池のほとりに行幸された。
それから九年を経て、建久五年甲寅閏八月二十七日に地震があった。
