高祖父野本将監《割書：諱不知｜生国武蔵》代々武蔵国野本ノ郷ノ地侍ニテ
同国岩附ノ城主太田三楽エ相勤候《割書：其節ヨリ称号｜野本ト相改》下総国鴻ノ
台合戦ノ時武功有之其以後三楽養子北条十郎エ仕ヘ小
田原没落ノ砌 秀康公エ小田原於御陣小屋将監并其子
右近父子一所ニ被召出伏見ニ定詰相勤候右近正則其以
後肥前国名護屋御出張ノ節相勤 忠直公御代大坂両度
ノ御陣御供仕候元和元年五月七日桜ノ門エ乗入之時ニ
御宿勘兵衛正倫ヲ見懸ケ詞ヲカハシ互ニ馬ヨリ下リ立
鎗ヲ合セ首ヲ取申候 両御所様於 御前御実検之節右
近手ヲ負候ト相見候由 権現様上意ニテ御手自御薬ヲ

当代 野本源左衛門恒高《本国武蔵 生国越前｜姓は平 本名は岩木》
高祖父野本将監《諱は不明｜生国武蔵》は代々武蔵国野本の郷の地侍で、
同国岩付の城主太田三楽に仕えていた《その節より称号を｜野本と改める》。下総国鴻の
台合戦の時に武功があり、その後三楽の養子北条十郎に仕え、小
田原没落の際、秀康公が小田原の御陣小屋において将監並びにその子
右近父子を一緒に召し出し、伏見に定詰として勤めさせた。右近正則はその後
肥前国名護屋御出張の節に勤め、忠直公御代の大坂両度
の御陣にお供した。元和元年五月七日桜の門に乗り入れた時に
御宿勘兵衛正倫を見かけ、言葉を交わし、互いに馬より下りて立ち
槍を合わせ首を取った。両御所様の御前での御実検の節、右
近は手を負ったと見受けられたが、権現様の上意にて御手ずから御薬を
頂戴し、その上判金二枚、菖蒲革の御袴を拝領した。台徳院様

Summoned to service in the 6th year of Kyōhō, Kanoto-Ushi year (1721) during Lord Yoshikuni's reign
Current Nomoto Genzaemon Tsunetaka 《Native province: Musashi Birth province: Echizen｜Family name: Taira Original name: Iwaki》
Great-great-grandfather Nomoto Shōgen 《Personal name unknown｜Native province: Musashi》 was for generations a local samurai of Nomoto village in Musashi Province,
serving Ōta Sanraku, lord of Iwatsuki Castle in the same province 《From that time, changed｜his family name to Nomoto》. During the Battle of Kōnodai in Shimōsa Province,
he distinguished himself in battle, and afterward served Hōjō Jūrō, the adopted son of Sanraku. When Odawara
fell, Lord Hideyasu summoned both Shōgen and his son
Ukon together at the field headquarters in Odawara, and they served as regular attendants in Fushimi. Ukon Masanori thereafter
served during the expedition to Nagoya in Hizen Province, and accompanied Lord Tadanao during both
Osaka campaigns. On May 7th of the 1st year of Genna (1615), when charging into Sakura Gate,
he encountered Oyado Kanbei Masatomo, exchanged words with him, both dismounted from their horses and engaged
in spear combat, taking his head. During the head inspection before both Their Lordships, it appeared that U-
kon had been wounded, but by the gracious command of His Majesty Gongensama (Tokugawa Ieyasu), he personally bestowed medicine,
