先祖之儀畠山庄司次郎重忠嫡男秩父六郎重清苗裔之由
申伝候近世朝倉家ニ罷有当国一乗谷ニ数代居住仕大月
村ヲ領知仕候故大月ヲ称号ト仕候志比谷市野々村ノ奥
ニ于今館跡有之候高祖父大月備中守景秀初名大月三郎
左衛門ト申者於朝倉家家禄千石領知仕候朝倉家滅亡之
後二君之存念無之北庄ニ居住仕以後剃髪仕候而大月善
隆ト号シ医術ヲ業ト仕候夫ヨリ相続大月寿斎其子大月
斎庵其子大月玄通ト申候ハ宗庵父ニテ候

吉邦公の御代、享保六年辛丑年（1721年）に召し出される。
現当主 大月宗庵順応（本国・生国ともに越前｜姓は平）
先祖の件について、畠山庄司次郎重忠の嫡男である秩父六郎重清の苗裔であると申し伝えられている。近世には朝倉家に仕え、当国の一乗谷に数代にわたって居住し、大月村を領知していたため「大月」を称号とした。志比谷市野々村の奥に今でも館跡が残っている。高祖父の大月備中守景秀（初名は大月三郎左衛門）という者は、朝倉家において家禄千石を領知していた。朝倉家滅亡の後は二君に仕える意思はなく、北庄に居住し、その後剃髪して大月善隆と号し、医術を業とした。その後継承して大月寿斎、その子が大月斎庵、その子が大月玄通といい、これが宗庵の父である。

Summoned to service in the sixth year of Kyōhō, kanoto-ushi year (1721), during the reign of Lord Yoshikuni.

Regarding his ancestors, it is said that they are descendants of Chichibu Rokurō Shigekiyo, the eldest son of Hatakeyama Shōji Jirō Shigetada. In recent times, they served the Asakura family and lived for several generations in Ichijōdani in this province, holding dominion over Ōtsuki village, hence adopting "Ōtsuki" as their family name. The remains of their residence can still be found in the depths of Shihi-tani Ichinonomura. His great-great-grandfather, Ōtsuki Bitchū-no-kami Kagehide (originally named Ōtsuki Saburō Zaemon), held a family stipend of 1,000 koku under the Asakura family. After the fall of the Asakura family, having no intention of serving two lords, he resided in Kitanoshō, later took the tonsure and called himself Ōtsuki Zenryū, practicing medicine as his profession. Subsequently came Ōtsuki Jusai, then his son Ōtsuki Saian, then his son Ōtsuki Gentsu, who was Sōan's father.
