曾祖父吉田又兵衛ト申者 秀康公御供仕結城ヨリ越前
エ罷越相勤其子吉田又兵衛儀越後守様御共仕越後エ罷
越相勤以後病身ニ罷成御暇申請京都エ罷越病死仕候又
兵衛儀 御家ニ相勤候海福瀬左衛門ト相婿ニテ候故越
後ヨリ浪人ニテ京都エ罷越候砌妻子ヲハ越前エ指越置
申候又兵衛嫡子権之丞儀ハ父ノ遺言ニテ医師ニ罷成京
都ニ罷有以後当国エ罷越田代養山弟子ニ罷成吉田益庵

吉邦公の御代、正徳三年癸巳年（1713年）に召し出された。
現在の吉田玄盛光竹《本国は下総、生国は越前、姓は不明》
曾祖父の吉田又兵衛という者は、秀康公にお供して結城より越前へ参り、勤仕した。その子の吉田又兵衛は、越後守様（忠直）にお供して越後へ参り勤仕したが、その後病身となり、お暇を申し請けて京都へ参り、病死した。この又兵衛は、御家に仕えていた海福瀬左衛門と相婿の関係であった。そのため、越後より浪人として京都へ参った際、妻子は越前に残し置いた。又兵衛の嫡子権之丞は、父の遺言により医師となり、京都にいたが、その後当国（越前）へ参り、田代養山の弟子となり、吉田益庵と名乗って町医者をした。その子が現在の玄盛光竹である。

Summoned to service in the third year of Shōtoku, year of the water snake (1713), during the reign of Lord Yoshikuni.
The current Yoshida Genmori Kōchiku 《Home province: Shimōsa, birth province: Echizen, family name: unknown》
His great-grandfather, called Yoshida Matahei, accompanied Lord Hideyasu from Yuki to Echizen and served there. His son, Yoshida Matahei, accompanied Lord Echigo-no-kami (Tadanao) to Echigo and served there, but later became ill, requested leave, went to Kyoto, and died of illness. This Matahei was related by marriage to Kaifuku Sazaemon, who served the domain. Therefore, when he went to Kyoto as a masterless samurai from Echigo, he left his wife and children in Echizen. Matahei's eldest son Gon-no-jō, following his father's deathbed instructions, became a physician and was in Kyoto, but later came to this province (Echizen), became a disciple of Tashiro Yōzan, took the name Yoshida Ekian, and practiced as a town doctor. His son is the current Genmori Kōchiku.
