現代語訳
御医師方が全員お集まりになり、一人ずつ御診察したところ、今の昼時より、大変危篤な状態である旨を御家老に申し伝えた。交泰院の薬を中止することになったため、御医師中で御相談の上、内田玄寿老の薬を差し上げることになった。
一 同日、右同様の理由により、江戸表より早飛脚が到着したことにより、すべての社寺に御祈祷を仰せ付けられた。
一 同日、右同様の理由により、江戸表より早駆御徒飛脚山内左助が到着したため、御家老より御機嫌を伺うため、御用人稲垣安右衛門泰久、岡三郎右衛門経忠、御医師有賀玄哲を江戸表に派遣し、今日福井を出立した。
一 十月二十日、右同様の理由で、御様体がなお一層お勝れにならないため、
御一門様並びに御身近の御大名、その外御旗本中にも御知らせがあった。
一 同日、右同様の理由により、松平大膳太夫宗廣様がお見舞いされ、御医師中に御療治のことをお頼みになった。内田玄寿老が辞退されたため、数原通玄老にお薬を変更された。
一 同日、御医師武田長春院がお越しになった。
宗矩様の御病気が大変危篤である旨が上聞に達したところ、大変御心配されており、一家の者は油断なく療養に心をつけるよう、詰め合わせている者に長春院より申し聞かせるようにとの上意の趣旨の御達しがあった。

All the court physicians assembled and each conducted individual examinations. From around noon today, they reported to the chief retainer that his condition had become extremely critical. As the medicine from Kōtai-in was discontinued, after consultation among the physicians, medicine from Uchida Genju-rō was administered.
