一、閏四月二十六日頃、初姫様が御不快（病気）になられたため、少々河野仙壽院の薬を御服用になった。
一、同月二十七日、使者により例年通り大納言様へ菖蒲の御兜を献上した。
一、五月朔日、御不快のため御登城をお断りした。
一、同月二日、端午の祝儀として使者により例年通り御帷子・御単を献上した。
一、同月四日、御不快のため東叡山辺の出火の際に御出馬が困難である旨を、御用番本多中務大輔忠良殿へ使者として御留守居浅井源左衛門政之により御届けした。
一、同月五日、御不快のため御登城をお断りした。
一、同月六日、使者により例年通り御国産の初鯖を献上した。
一、同月八日、初姫様の御病気が御大切（重篤）になったため、御見舞いのため大和守様、越後守様、出羽守様、松平大膳大夫様がお出でになった。
一、同日、右の件について御養生も御叶いにならず、夜五半時（午前3時頃）に御逝去された。御法号は照光院殿芳誉源真圓智大〔以下略〕。
一、同日、右の件により御忌服の儀について左の通り御書付により、御用番本多中務大輔忠良殿へ使者として御留守居荒川十右衛門通朗により御届けした。
