私儀親之名ニ御座候ニ付十三郎と改名仕度

右丁代印形ハ無之候得共扣置申候三月廿一日差上候
此御願三月廿二日夕方相済申候右二付御宗門帳ハ
矢張多八郎ニ而差置申候判者御願も同刻被 仰付候
乍恐
私儀栄蔵と申し候而御水役相勤鍛冶職
仕候處此度伝十郎与改名仕候ニ付何卒御水役
札御書替被成下候様奉願上候以 御慈悲奉願上候通
被 仰付被下置候ハヽ難有仕合奉存候御事

以上の件につき、お願い申し上げます。

上記の通り三月六日、月番のいせやに願書を提出いたしました。
恐れながら
私の名前が親の名前と同じでございますので、十三郎と改名いたしたく存じます。つきましては、お願い申し上げます。

上記の町代の印鑑はございませんが、控えとして記録しております。三月二十一日に提出いたしました。
この願いは三月二十二日夕方に処理が完了いたしました。これに関して宗門帳は
やはり多八郎のままにしておきます。判決も願いと同時にお許しをいただきました。
恐れながら
私は栄蔵と申しまして、水役を務め鍛冶職を
しておりますが、この度伝十郎と改名いたしますので、どうか水役の
札をお書き替えくださいますようお願い申し上げます。ご慈悲をもって、お願い申し上げます通り
お許しくださいますれば、ありがたき幸せと存じます次第
