## 【鍼灸経・医門方より — 年齢による制限、発汗法、処方】 ### 鍼灸経より 『針灸経』に云う:十歳の小児と七十歳の老人は、鍼(はり)を用いてはならない。灸(きゅう)および甘薬(かんやく、甘味のある薬)が宜しい。 ### 医門方より 『医門方』に云う:大法(おおむね)として、春・夏には発汗(はっかん)が宜しい。 およそ湯薬(とうやく)を服して汗を発し、病に中(あた)れば、すみやかに止めよ(服薬を止めよ)。必ずしも一剤(ひとつの処方)を飲み尽くさなくともよい。 およそ汗を発するには、手足にくまなく(あまねく)〔蟄(ちつ):「直立反」の反切で、意味は「蔵(かくれる)」〕津液(しんえき)を巡らせ、体全身を一時(いっとき)に潤わせることが最もよい。ただし流れ離れるように激しい雨のごとく(大量に)出るほどには要しない。粉(こな)をもって体に磨り塗り、風・冷(ひやかなる冷気)に当たることなかれ。 およそ大いに汗が出た後、再びその後(に)脈が洪大(こうだい)となり、形(ありさま)が瘧(おこり)のようで、一日に二度発する(発作がある)場合、汗が出ればすなわち解(かい)する。さらに桂枝麻黄湯(けいしまおうとう)を与えよ。 --- **桂枝麻黄湯(けいしまおうとう)** - 麻黄(まおう)〔訓:くれのはしかみ、またはあまな、またはかつねくさ〕 四両(節を去る) - 桂心(けいしん) 二両 - 甘草(かんぞう)〔甘き草〕 一両〔炙る〕 - 杏人(きょうにん)〔からもものさね〕 八十枚(尖りを去る) 水七升をもって、先ず麻黄を三沸(みたび沸かし)、沫(あわ)を払い去り、諸薬を入れ、煮て二升半を取る。三回に分けて服せよ。服と服の間は七・八里(の歩行時間ほど)を隔て、〔覆(おお)う〕(身体を温かく覆う)。汗(をかかせる)。粥(かゆ)を啜(すす)る必要はない。余は桂枝(けいし)の法の如くにせよ。 --- およそ汗を発した後、汗がついに漏れ出て止まらず、その人が風を悪み(にくみ)、小便(が出ること)難しく、四肢(ししゅ)がわずかに急(きゅう、こわばり)となり、屈伸(かがめのばすこと)が難しい場合は、桂枝(けいし)に附子(ぶし)を加えた湯(桂枝加附子湯)がこれを主(つかさど)る。 --- **桂枝加附子湯(けいしかぶしとう)** - 桂心(けいしん) 三両 - 芍薬(しゃくやく)〔えびすくすり〕 三両 - 甘草(かんぞう) 二両〔炙る〕 - 生姜(しょうきょう) 三両 - 大棗(たいそう) 十二枚〔群(つむぐ)・切(刻む)〕 水七升をもって微火(とろ火)にて煮て三小升(さんしょうしょう)を取り、滓(かす)を去る。温かいまま三回に分けて服せよ。湯を服し終えたら、さらに一升の熱い粥を啜り、もって薬力(やくりょく)を助けよ。温かく覆い(体を覆って)、(微かに)汗ばませよ。(汗が)出ていなければ、また前のとおりに服せよ。汗が出ない者は、その間(のあいだ)半日ほどより、乃ち差(い)ゆるのを覚ゆるであろう。明晨(みょうしん、翌朝の早い時刻)にその病を候(うかが)え……

## [From the *Shinkyū-kyō* and *Imon-hō* — Age-based Restrictions on Needling, Methods of Inducing Perspiration, and Prescriptions] ### From the *Shinkyū-kyō* (Classic of Acupuncture and Moxibustion) The *Shinkyū-kyō* says: Children of ten years of age and the elderly of seventy years of age must not receive acupuncture needling. Moxibustion (*kyū*) and sweet medicines (*kanyaku*) are appropriate for them. ### From the *Imon-hō* (Medical Formulary) The *Imon-hō* says: As a general principle (*ōmune*), in spring and summer it is appropriate to induce perspiration. In general, when taking an herbal decoction (*tōyaku*) [to promote sweating]: if the sweating is induced and hits the mark [i.e., effectively addresses the illness], stop [taking the medicine] at once. It is not necessarily required to finish the entire prescribed dose (*sai*, one full course of the formula). In general, when inducing perspiration, it is best to have the fluids and humours (*shin'eki*, i.e., body fluids) circulate throughout, reaching the hands and feet evenly, moistening the entire body all at once. However, it is not necessary for the sweat to pour forth like a streaming rainstorm. Apply powder by rubbing it onto the body, and do not expose [the body] to wind or cold. In general, after a great sweat has broken out and then subsequently the pulse becomes *kōdai* (surging and large), and the appearance [of the condition] resembles that of malaria (*okori*), recurring twice in one day — when sweat breaks out [again], the condition will resolve. [In this case,] administer *Keishi Maō-tō* (Cinnamon Twig and Ephedra Decoction) once more. --- **Keishi Maō-tō 桂枝麻黄湯 (Cinnamon Twig and Ephedra Decoction)** - Ephedra (*maō*) [vernacular names: *kure no hashikami*, or *amana*, or *katsunekusa*]: 4 *ryō* (nodes removed) - Cinnamon heartwood (*keishin*): 2 *ryō* - Licorice (*kanzō*) [lit. "sweet herb"]: 1 *ryō* (roasted/processed) - Apricot kernels (*kyōnin*) [lit. "seeds of the Chinese apricot"]: 80 pieces (tips removed) Using 7 *shō* of water, first boil the ephedra until it has come to three boils; skim off and discard the foam; add the remaining medicines; boil down and obtain 2½ *shō*. Divide into three doses and take. Allow seven to eight *ri* [of travel time] between each dose. [Cover the body to keep warm.] [This will induce] perspiration. It is not necessary to sip (*susuru*) rice gruel (*kayu*). For the remainder [of the treatment], follow the method of *Keishi* (Cinnamon Twig Decoction). --- In general, after perspiration has been induced and the sweat subsequently continues to leak out without stopping — and the patient avers to wind (*kaze o nikumu*), finds urination difficult, has slight stiffness (*kyū*) in the four limbs, and finds it difficult to flex and extend them — *Keishi-ka-Bushi-tō* (Cinnamon Twig Decoction plus Aconite) governs this condition. --- **Keishi-ka-Bushi-tō 桂枝加附子湯 (Cinnamon Twig Decoction with Aconite)** - Cinnamon heartwood (*keishin*): 3 *ryō* - Peony (*shakuyaku*) [vernacular: *ebisu-kusuri*]: 3 *ryō* - Licorice (*kanzō*): 2 *ryō* (roasted/processed) - Fresh ginger (*shōkyō*): 3 *ryō* - Jujube (*taisō*): 12 pieces (crushed and sliced) Using 7 *shō* of water, cook over a gentle flame (*toro-bi*) and obtain 3 small *shō*; remove the dregs (*kasu*). Take warm, divided into three doses. After taking the decoction, it is necessary to further sip one *shō* of hot rice gruel to help augment the power of the medicine. Cover warmly [to keep the body warm] and allow [the patient] to perspire gently. If [the sweating] does not appear, take [the medicine] again as before. For those in whom sweating does not occur, from about half a day's interval [after taking the medicine], one will begin to feel recovery (*iyuru*). Observe the illness the following morning (*myōshin*)…
