次第に重り十月八日今日は命危うく危く相成
仙太郎辰蔵と呼わる故側へ参りけれは
手に取付是迄は大きに世話に相成候へへとも
養生も叶わす最早湯水も通らねは
次第に弱る計也願わくは弐人の衆達者
て国へ帰るなら子供の事を頼そと両
眼に涙浮へて申けり誠に見る目も哀成
璧唐にも天竺にも耆婆扁鵲（きばへんしやく）は昔の事

今は末世におとろへて命を奨医師も
なく寿命を辿る薬もなく去は逃れぬ
定業の無道の風に松之助死ぬる命は是
悲もなし跡弐人生なからへて居ながらも
片腕とらるゝ心地也夫ゟ四五丁脇に並子
と云所陣屋あり御検使大役人壱人上下
十弐人にて来其時三人の名前を書て差
出せば遠江国をわんかんとよむ仙太郎せん

次第に重くなり、十月八日、今日は命が危うく危険な状態になったので、仙太郎と辰蔵が呼ばれて側に参ったところ、手を取って「これまでは大変お世話になりましたが、養生も叶わず、もはや湯水も通らないので、次第に弱るばかりです。願わくは二人の衆が達者で国へ帰るなら、子供のことを頼みます」と両眼に涙を浮かべて申しました。誠に見る目も哀れで、唐にも天竺にも耆婆や扁鵲は昔のことです。

今は末世に衰えて、命を助ける医師もなく、寿命を延ばす薬もなく、さては逃れぬ定業で、無情の風に松之助が死ぬる命はこれも悲しいことではありません。後に二人が生き長らえていながらも、片腕を取られる心地です。それから四、五丁脇に並子という所に陣屋があり、検使の大役人一人が上下十二人で来て、その時三人の名前を書いて差し出すと、遠江国を「をわんかん」と読み、仙太郎を「せん」
