一 青山大膳亮様がご病気につき、療養のため月代（髪剃り）をしたい旨をお願いの通り仰せ出された由
一 本庄甲斐守様の曽祖母様がご病気のため、療養として相州塔ヶ沢へ三回の湯治にお越しになり、お帰りの際に江島・鎌倉へ参詣したい旨、去る十一日に願書を差し出したところ、お願いの通り仰せ出された由

One: Regarding Lord Aoyama Daizen-no-suke's illness, his request to have his monthly head shaving (tsukishiro) for recuperation has been approved as requested.
One: Regarding Lord Honjō Kai-no-kami's great-grandmother's illness, she wishes to go to Tōnozawa in Sagami Province for hot spring therapy three times for recuperation, and on her return journey, she wishes to make pilgrimages to Enoshima and Kamakura. A petition was submitted on the 11th, and it has been approved as requested.
