一 同10年（1839年）己亥11月10日、御内沙汰【場所：江戸】表在番中、医業随身方等あれこれと御世話も成され御座候ところ、修行に心を留めず雑事に取り紛れ、御沙汰筋等閑に相心得、ここ元でも仰せ付けられた御用筋を忘却しないけれども、これもまた同様の義共があり、その上在勤中は中村中書用弁のみに専心し、度々外出して御用向を怠る義共が数度あり、かつ御膳番中に聞いた義共を軽忽に相心得、不慎の義共があり、殊に身分を顧みず寛怠の大言共に及び交通にも候の段、品々軽からざる次第共を押し立て急度御糺明を及ぼされては身分相立ち難く、これにより此度は格別の御憐愍をもって御沙汰に及ばれず候、恐れながらこちらとも申し談じたところ、何分打ち捨て難く、内分取締に及び沙汰候の条、早々先非を悔い御奉公に相励み候様致したき事候。もっとも伊東玄棟・藤田祐甫が心付け助言の義に及び沙汰候条、いささかも違背無くこれを相守り、勤謹専要と相心得るべく候

Moreover, during his service he focused solely on clerical work for Nakamura Chūsho, frequently went out and neglected official duties on numerous occasions, and carelessly treated matters heard during meal service, showing imprudent behavior. Particularly, disregarding his station, he engaged in idle boastful talk and inappropriate social interactions. Given these various serious matters, if strict investigation were conducted, his position would become untenable. Therefore, this time, through special compassion, formal punishment was not imposed. Humbly, after consultation with our side, since the matter cannot be dismissed, internal disciplinary action was taken. He should quickly repent his past wrongs and devote himself to loyal service. Furthermore, Ito Gento and Fujita Sukesuke were instructed to provide guidance and advice, which he must follow without any violation, understanding that diligent service is of utmost importance.
