其方儀家業未熟之段者申迄茂無之処祖父中伊奈
以来 御方向成上候ニ付以 御慈悲
御身近ニ茂被 召仕候然処身分冥利をも不相弁人命
ニ茂拘リ候大切之家業を投遣リニ致置都而之心得方ニも
毎度不束之儀共不少候付心得方等品々
御尋有之刻者一々虚言偽を以申掠メ候心底
上を蔑み如ニ相心得不埒至極ニ付此方共其外ゟ茂及
数度致教戒候而も聊以改心之躰無之近頃手近キ薬名
之文字又者針灸之穴所等 御尋有之候而も更ニ不相
心得右体ニ而茂相済候儀ト其分ニ而罷在候心底言語同
断之事ニ候既ニ先年為修行御暇相願久々他郷ニ罷在候
詮茂不相立一々被及 御糺明候而者不軽次第ニ付猶又出格之
思召ニ而格式被 召上向後御茶道並御用人支配ニ被
仰付候本道外科其外 御座右之御用向等者一切不被
仰付誠ニ按摩導引一通リ之儀者時ニ取可被 仰付候間
御膳番中差引ニ随ひ 御殿江相詰坊主部屋ニ控罷在
御沙汰次第御療治差上可申候斯迄之
御仁恕茂不相弁此上茂不埒之義有之候はゝ其節者厳

一 嘉永3年（1850年）庚戌4月18日、亡父養悦跡式として高6人扶持を下され、御供番格表医を仰せ付けられ、御用人支配とされた
一 同7年（1854年）甲寅8月13日、御繰合により江戸引越を仰せ付けられ、同月29日江戸着、翌月末日、御家中療養に精出して相勤めた彼是をもって、無足医を仰せ付けられ、1人扶持御加増、高7人扶持を成し下された
一 同年9月3日、安田俊蔵養方の妹を妻にしたいとの願いの通り仰せ出された
一 安政4年（1857年）丁巳4月8日、御用番御宅にて左の通り仰せ渡された
「その方の家業が未熟という段は申すまでもないところ、祖父中伊奈以来御方向き成し上げ候により、御慈悲をもって御身近にも召し仕えさせられた。しかしながら身分の冥利も相弁えず、人命にも関わる大切の家業を投げ遣りにし置き、すべての心得方にも毎度不束の儀共少なからず候により、心得方等品々御尋ねある刻は一々虚言偽りをもって申し掠め候心底、
上を蔑み如きに相心得、不埒至極により、こちらともそのほかからも数度教戒を致し候えども、いささかも改心の体無く、近頃手近き薬名の文字または針灸の穴所等御尋ねある候えども更に相心得ず、右の体にても相済み候儀とその分にて罷り在り候心底、言語同断の事に候。既に先年修行のため御暇相願い、久々他郷に罷り在り候詮も相立たず、一々御糺明に及ばれ候えば軽からざる次第により、なお又出格の思し召しにて格式を召し上げられ、向後御茶道並に御用人支配に仰せ付けられ候。本道外科そのほか御座右の御用向等は一切仰せ付けられず、誠に按摩導引一通りの儀は時に取り仰せ付けらるべく間、御膳番中差引きに随い御殿へ相詰め、坊主部屋に控え罷り在り、御沙汰次第御療治差し上げ申すべく候。これまでの御仁恕も相弁えず、この上も不埒の義これあり候わば、その節は厳格の御取扱いあるべき条、その期に至り後悔無きよう今より覚悟致すべく候。これにより逼塞以上の慎みも仰せ付けらるべきところ、別段御含み筋これあり、勤仕のほか慎み罷り在り候よう仰せ付けられた」

- Kaei 3 (1850), 4th month, 18th day: Following his deceased father Yōetsu's position, he was granted a stipend of 6 person-rations, appointed as physician of attendant rank, and placed under the supervision of senior officials.
- Same 7th year (1854), 8th month, 13th day: Due to administrative arrangements, he was ordered to relocate to Edo. He arrived in Edo on the 29th of the same month, and by the end of the following month, due to his diligent service in treating retainers' medical needs, he was appointed as stipendless physician and granted an increase of 1 person-ration, bringing his total to 7 person-rations.
- Same year, 9th month, 3rd day: His request to marry the sister of Yasuda Shunzō's adoptive family was approved as requested.
- Ansei 4 (1857), 4th month, 8th day: The following was declared at the senior official's residence:
"That your medical practice is immature goes without saying, but since your grandfather Chu-Ina has served our house faithfully, out of compassion you have been employed in close service. However, you do not understand the privileges of your position, neglect the vital practice that involves human lives, and consistently show negligent behavior in all aspects of your duties. When questioned about various medical knowledge, you repeatedly respond with lies and deceptions.
Your attitude of despising superiors is extremely improper. Despite repeated admonishments from us and others, you show no signs of reform. Recently, when asked about common medicine names or acupuncture points, you show complete ignorance, yet remain complacent thinking this is acceptable—such an attitude is inexcusable. Previously, when you requested leave for study and spent a long time in other provinces, nothing came of it. Upon detailed investigation, these are serious matters. Therefore, through extraordinary consideration, your rank is being reduced, and henceforth you are appointed to tea ceremony duties under the supervision of senior officials. You will not be assigned any internal medicine, surgery, or other close advisory medical duties. Only simple massage and physical therapy may occasionally be assigned. You shall attend the palace according to meal service schedules, wait in the monk's quarters, and provide treatment only upon specific orders. If you fail to understand this benevolence and continue improper behavior, strict measures will be taken at that time. You must prepare yourself from now to avoid future regret. While confinement or greater punishment should be imposed, due to special circumstances, you are ordered to remain in disciplinary restraint except when performing duties."
