科をくひかなしひそれより御主ぜずきり
しとの御おきてをたもち奉るべきとのか

現代語訳
罪を悔い悲しみ、それより御主イエス・キリストの御掟を保ち奉るべきとの覚悟を以てこの秘跡を受くること肝要なり。
弟子：この秘跡をばいかように授け給うぞ。
師：これを授かる人の頭か、せめてその人の身の上に水をかけると共にこの文を唱ゆべし。いかに○ペトロ、パウロとなり共名を付けて○それがしデウスは父と子と聖霊の御名を以て汝を洗い奉る也。アーメンと云うべし。これを経文の唱えには○何の名なり共付けて後☩エゴ○テ○バプティーゾ○イン・ノーミネ○パートリス○エット○フィーリイ○エット○スピーリトゥス○サンクティ○アーメンと云うなり。
英語訳
sins, repent and grieve for them, and from then on be resolved to keep and observe the commandments of our Lord Jesus Christ - it is essential to receive this sacrament with such preparation.
Disciple: How is this sacrament administered?
Master: Water should be poured on the head of the person receiving it, or at least on some part of their body, while reciting these words: "Whether the name be Peter, Paul, or whatever name is given, I, [name], God washes you in the name of the Father, and of the Son, and of the Holy Spirit. Amen." When reciting this in Latin scripture, after giving whatever name, one says: "Ego te baptizo in nomine Patris et Filii et Spiritus Sancti. Amen."
