乍恐以口上書奉願上候御事
私儀病身ニ御座候処男子無御座候ニ付中沢郷
北福地村嘉十倅伝治と申者内縁も御座候ニ付
此度貰請養子ニ仕跡式相続為仕度内談
仕候仍之引取申度奉願上候
右之趣以 御慈悲奉願上候通被為 仰付

恐れながら口上書をもって願い上げる件について
私は病身でございますが、男子がおりませんので、中沢郷
北福地村の嘉十の息子で伝治と申す者と内縁関係もございますので、
この度もらい受けて養子にし、跡継ぎをさせたく内談
いたしましたので、引き取りたくお願い申し上げます。
右の件について、ご慈悲をもってお願い申し上げる通りに仰せ付け
いただけましたなら、ありがたい仕合わせと存じます。以上
