一 同四年丁卯（1747年）八月二十八日、松平美濃守様の浪人当時の町医師久志本三庵の娘を妻にしたいと願い出て許可される
一 同五年戊辰（1748年）五月十三日、相州伊勢原にいる叔父源藤忠八が代々郷士であり、その名跡相続の件につき永の御暇を願いの通り賜る。奉公の件は差し支えなし。ただし両御屋敷への出入りはしてはならない
小山田新十郎
頼以君の御代
一 文化十二年乙亥（1815年）三月二十二日、召し抱えられ格式御供番として高二十二俵二人扶持を賜り、御祐筆を仰せ付けられる。別段御役中として金二両二分一人扶持を賜る
一 同十五年戊寅（1818年）正月晦日夕方、不如意のため今後も御奉公を勤めたく、近辺の町家の子供に手本を与えて書道を指南し、追々人数も増えて行き届かないこともあり、外宅を願い出る趣旨では難しい筋のところ、何分御間に支障をきたさないよう勤めると申し立てたので、願いの通り仰せ出される。これにより御宛行の半高を差し上げたいとの趣旨であったが、格別の思し召しをもってそのまま賜る
頼寧君の御代
一 文政五年壬午（1822年）十二月二十七日、平日の勤務が等閑で、その上身持ちが不束との趣が聞こえ不都合につき、御宛行を召し上げられ永の御暇を賜る。上下御屋敷はもちろん御在所近辺への徘徊を堅く禁止し、奉公は差し支える

- Enkyō 4, year of the Fire Rabbit (1747), 28th day of the 8th month: Request approved to marry the daughter of Kushimoto San'an, a town physician during the time when Matsudaira Mino-no-kami-sama was a masterless samurai
- Enkyō 5, year of the Earth Dragon (1748), 13th day of the 5th month: Regarding his uncle Gentō Chūhachi in Isehara, Sagami Province, who has been a village samurai for generations, granted permanent leave as requested for matters concerning succession to that family name. - Bunka 15, year of the Earth Tiger (1818), last day of the 1st month, evening: Due to financial difficulties, wishing to continue loyal service, requested to give calligraphy lessons to children of nearby townspeople with the number of students gradually increasing and sometimes becoming unmanageable. While external residence would normally be difficult to approve, since he pledged to serve without interfering with his duties, his request was granted as submitted. Regarding this, he wished to surrender half his stipend, but through special consideration it was allowed to remain as is
During the reign of Lord Yoriyasu
- Bunpō 5, year of the Water Horse (1822), 27th day of the 12th month: Due to negligent performance of daily duties and reports of misconduct, his stipend was confiscated and permanent leave granted. Strictly prohibited from loitering around both upper and lower residences as well as the vicinity of the domain residence, and service obligations suspended
