一 同二年庚戌二月六日 御徒士に御取り立て、高十八俵二人扶持。御家老御年寄書役、御用人書役はこれまでの通り。
一 同三年亥七月五日 厄介の件について願い済み、大坂にて。
一 同年七月 片倉村久左衛門の従弟片岡団太、当時加藤主計様の若党を相勤めている由来もあることから、厄介として仕え、伊沢直右衛門の養子となることを願い済み。

一 享和二年壬戌七月十三日 医道に心掛けるため、五ヶ年の間御宛行を差し上げ御暇を下し置き、修行したい旨、願いの通り仰せ付けられる。
一 文化三年丙寅八月二十八日 医業修行五ヶ年の御暇を相願い、この度帰参したため、御徒士元席に成し下され、高十八俵二人扶持を下される。御家中療治を仰せ付けられる。
一 同四年丁卯四月九日 御含みの筋があり、高のうち二俵を召し上げられ、御徒士格御次番を仰せ付けられる。医業を御取り上げ、還俗閉門を仰せ付けられる。
一 同年五月三十日 閉門を御免される。
一 同年六月十五日 出奔により家断絶。

- Same year, 7th month:片岡団太, a cousin of Katakura village's Kyūzaemon, who was currently serving as a retainer to Lord Katō Kazue, was accepted as a dependent and his request to become an adopted son of Izawa Naouemon was approved.

- 10th year (Kansei 10, 1798), 1st month, 15th day: He inherited his late father Daisuke's position with a stipend of eighteen hyō and two-person support, and was made a foot soldier.
- Kyōwa 2 (1802), 7th month, 13th day: As he wished to pursue medical studies, he was granted five years' leave, surrendering his stipend allocation for training purposes, as requested.
- Bunka 3 (1806), 8th month, 28th day: Having completed five years of medical training during his leave and now returning, he was restored to his former foot soldier position with a stipend of eighteen hyō and two-person support, and appointed to provide medical treatment within the domain.
- 4th year (Bunka 4, 1807), 4th month, 9th day: Due to certain implied circumstances, two hyō were deducted from his stipend, he was appointed to foot soldier rank rotating guard duty, his medical practice was terminated, and he was ordered to return to secular life and house confinement.
