之訳柄を以子孫之者永々 御目見苗字一刀御免被仰付
安政四巳年忰庄蔵御仕送り役見習奉蒙 仰御扶持方
頂戴仕奉蒙 御重恩尚又御衣服御上下度々幷御品物
等数度拝領仕去巳年三之丸内杖御免被 仰付重畳
冥加至極難有仕合ニ奉存候尚此上出精仕可奉報 御重恩
心願御座候得共元来近眼之処近頃別而不服ニ相成其上
持病之疝癪折々相発難儀仕候御時節柄奉恐入候得共
右御役儀御赦免被成下置候様奉願上候
右之趣以 御慈悲ヲ奉願上候通被為 仰付被下置候者
難有仕合ニ奉存候以上

現代語訳
この由緒により、子孫の者が永代にわたって御目見得、苗字帯刀の御免許をいただきました。安政四年巳年には息子庄蔵が御仕送り役見習いを仰せつかり、御扶持方をいただき、御重恩を蒙りました。なお御衣服や御上下を度々、並びに御品物等を数度拝領いたし、去る巳年には三之丸内での杖の御免許をいただき、重ね重ね冥加至極ありがたい仕合いと存じております。なおこの上は出精して御重恩にお報いしたいと心願しておりますが、元来近眼であるところ、近頃は特に具合が悪くなり、その上持病の疝癪が折々発症して難儀しております。御時節柄恐れ入りますが、
右の御役儀の御赦免をお願い申し上げます。
右の趣旨について、御慈悲をもってお願い申し上げます通りにお仰せつけいただけましたら、ありがたい仕合いと存じます。以上

英語訳
Due to this hereditary connection, my descendants have been permanently granted audience with superiors and permission to bear family names and wear swords. In the fourth year of Ansei (1857), year of the Snake, my son Shōzō was appointed as an apprentice messenger and received stipend support, benefiting from great benevolence. Furthermore, I have received clothing, formal court dress repeatedly, as well as various goods on numerous occasions. In the previous Snake year, I was granted permission to carry a walking stick within the Sannomaru compound - truly an extremely fortunate blessing for which I am deeply grateful. I have wished to work diligently to repay this great benevolence, but I have been nearsighted from birth, and recently this condition has worsened considerably. Moreover, my chronic colic attacks occur frequently, causing me great difficulty. I deeply apologize for making this request during these pressing times, but
I humbly request that you grant me release from the above-mentioned official duties.
Regarding the above matter, if you would graciously grant my request as I have humbly petitioned, I would consider it a most grateful blessing.
