内容：筑後・越後・信濃・上野・陸奥・出羽の右記の御嶽山両大権現の社頭その他が大破したため、修復の助成として右の五カ国での勧化を許可する。寺社奉行連印の勧化状を持参した社役人が来て、辰年正月から午年十二月までの三年間、御料・私領・寺社領・在町を巡行することになった。信仰する人々は物の多少によらず寄進すべき旨を、御料は御代官、私領は領主・地頭より触れ出すべきである。
