恐れながら口上書をもって願い上げます件
私は不調法者でございますが、思いがけず町代の御役目を仰せつかり、有難く存じ勤めておりましたところ、病気のため御役目を勤めることが困難と存じます。つきましては御役目の御免職を下し置かれますよう願い上げます。

I am an incompetent person, but unexpectedly I was appointed to the honorable position of town representative, which I gratefully accepted and have been serving in. However, due to illness, I find it difficult to carry out the duties of this position. Therefore, I humbly request that you grant me dismissal from this official position.
