私は不調法者でございますが、思いがけず町代の役職を仰せつかり、ありがたく存じ相勤めておりましたところ、病気のため役職を勤めることが困難と存じます。つきましては役職の免除をお許しくださいますようお願い申し上げます。

I am an incompetent person, but unexpectedly received the appointment to the position of town representative (machi-dai), which I gratefully accepted and have been carrying out. However, due to illness, I find it difficult to perform my duties. Therefore, I respectfully request that I be relieved of this position.
