現代語訳
恐れながら
私は不調法者でございますが、思いがけず丁代の役職を仰せつかり、ありがたい仕合わせと存じ相勤めておりましたところ、病気のため役職を勤めることが困難と存じます。つきましては赦免をお許しくださいますようお願い申し上げます。

右は二十八日に提出したため、役所へ東池上の用事があったついでに差し上げたところ、聞き済みとなった。

Respectfully,
I am an incompetent person, but unexpectedly received the appointment to the position of town representative (chō-dai), which I consider a grateful fortune, and have been carrying out these duties. However, due to illness, I find it difficult to perform my duties. Therefore, I respectfully request that I be pardoned and relieved of this position.
