恐れながら口上書を以って願い上げ奉る件
飯田藩御支配所嶋田村の治兵衛方へ私の弟

私は思いがけず組頭役を仰せ付けられ、有り難く存じて相勤めておりましたところ、
病気のため勤めることが困難と存じます。つきましては御赦免くださいますよう
願い上げ奉ります。

To the household of Jihei in Shimada Village under the jurisdiction of Iida Domain, I wish
to send my younger brother Tōichirō as an adopted son, as there is already
an informal engagement. Therefore, I humbly request permission.

Respectfully,
I was unexpectedly appointed to the position of group head, which I gratefully accepted and have been serving in, but
due to illness, I find it difficult to continue my duties. Therefore, I humbly request
to be relieved of this position.
