私共組合米三郎儀当七月四日夜女房幷娘弐人
家出いたし候に付此段御届申上候米三郎儀身持
不宜風聞も御座候間此上行先おゐて何様之義
仕出し候半も難計奉存候依之右壱家之者
御宗門帳外被成下置候様奉願上候

右願書十二日於御役所池田氏坂井氏え差上候処明日

現代語訳
恐れながら口上書をもってお願い申し上げます事
私共組合の米三郎の件について、当年七月四日夜、妻並びに娘二人が家出いたしましたので、この段をお届け申し上げます。米三郎は身持ちが良くないとの風聞もございますので、この上行く先において何様な事を仕出かすかも計り難く存じます。これにより右一家の者を宗門帳から除外していただきますようお願い申し上げます。

Regarding Yonezaburo of our group association, on the night of July 4th of this year, his wife and two daughters ran away from home, so we report this matter to you. As there are also rumors that Yonezaburo's conduct is improper, we cannot predict what kind of trouble he might cause wherever he goes. Therefore, we petition that this entire household be removed from the religious register.
