【口上書をもって恐れながら願い上げ奉る御事】
去る十月十日に、私の父・紋治は鍛冶職を営んでおりましたところ、亡くなりましたため、（父が所持しておりました）御鑑札をご返納申し上げたく存じます。これにより、お願い申し上げます。
以上の事情について、どうかご慈悲をもって、願い上げました通りにお許しいただけますならば、ありがたく幸甚に存じます。以上。

[A Petition Humbly Submitted in Written Form]
On the tenth day of last October, my father Monjirō, who had been practicing the trade of blacksmithing, passed away. Therefore, we wish to return his official trade license (鑑札) to the authorities. We hereby respectfully submit this petition.
