此症（このせう）に二種（ふたいろ）あり。《割書：予が》薬王円にて治（ぢ）する症（せう）は。脾胃（ひゐ）虚（きよ）し。食物（しよくもつ）
一度（ひとたび）脾（ひ）にをさまるといへども。半時（はんとき）か一時。あるひは一日二日めに残（のこ）らず
吐（は）き。痩（やせ）おとろふを翻胃（ほんゐ）といふ。膈（かく）にまぎれる症なり。甚（はなはだ）六かしき症（せう）な
れども。皆（みな）こと〴〵く治（ぢ）す。膈（かく）といふは食物（しよくもつ）胸（むね）に滞（とゝこふり）てさがらず。直（すぐ）に吐（はく）を
かくといふ。此／症（せう）は治（ぢ）せず用ゆることなかれ。膈（かく）は大人ばかりにて小児は
まれなり。翻胃（ほんゐ）は大小児ともにあり

○ 眼病（がんびやう）
眼目（がんもく）は五臓（ござう）の精華（はな）。一身（いつしん）の肝要（かんよう）の所にて。其／疾（やま）ひ七十二／種（しゆ）と
いへども。おそらくは脾（ひ）虚（きよ）し。五臓（ござう）おとろへて眼（め）かすみいたみ。雀目（とりめ）
或は白瞙入（ほしいり）。外瞕（うはひ）内瞕（そこひ）。其外／種々（いろ〳〵）と変（かは）り発（はつ）するなり。この
薬は根本（こんほん）の脾（ひ）を補益（とゝのゆる）ゆゑ。かすむ眼は勿論（もちろん）。雀目（とりめ）は四五服
現代語訳
あるいは吐いたり、また下痢をしたりする。この病気で熱が多くて湯水を好む者は、五苓散の煎じ汁で薬王円の丸薬を用いる。もしまた悪寒が強くて湯水を好まない症状は、理中湯の煎じ汁で薬王円を用いて治療することは神のようである。
五苓散の処方 猪苓、沢瀉、茯苓、桂枝、白朮 各等分
理中湯の処方 人参三分、甘草三分、乾姜七分、白朮五分
右の両方ともに呉茱萸三分を加えて煎じ汁一服に薬王円の丸薬二服ずつを用いる。煎じ方は常のとおりである。
○翻胃 かくという
この症状に二種類ある。私の薬王円で治療する症状は、脾胃が虚弱である。食物が一度脾に収まるといっても、半時間か一時間、あるいは一日二日後に残らず吐き、痩せ衰えるものを翻胃という。膈に紛らわしい症状である。非常に困難な症状であるが、皆ことごとく治る。膈というのは食物が胸に滞って下がらず、すぐに吐くものを膈という。この症状は治らないので用いてはならない。膈は大人だけにあって小児には稀である。翻胃は大人小児ともにある。
○眼病
眼目は五臓の精華、一身の肝要な所で、その疾患は七十二種類といわれるが、おそらく脾が虚弱で五臓が衰えて眼がかすみ痛む。雀目あるいは白翳入眼、外障内障、その他種々と変化して発症するものである。この薬は根本の脾を補益するゆえ、かすむ眼はもちろん、雀目は四五服で
英語訳
They may vomit or have diarrhea. For this disease, when there is much fever and the patient desires hot water, use Yakuo-gan pills with a decoction of Goreisan. If there is strong chills and no desire for hot water, treat with Yakuo-gan using a decoction of Richu-to - the cure is like divine intervention.
Goreisan Formula: Polyporus, Alisma, Poria, Cinnamon twig, Atractylodes - all in equal parts
Richu-to Formula: Ginseng 3 bu, Licorice 3 bu, Dried ginger 7 bu, Atractylodes 5 bu
For both formulas above, add 3 bu of Evodia fruit and use 2 doses of Yakuo-gan pills with one dose of the decoction. Prepare the decoction in the usual manner.
○ Stomach Reflux (Also called "kaku")
There are two types of this condition. The condition treated with my Yakuo-gan involves spleen-stomach deficiency. Even though food settles in the spleen once, after half an hour to one hour, or even after one or two days, everything is vomited up completely, causing emaciation and weakness - this is called stomach reflux. It is a condition that can be confused with "kaku." Though it is an extremely difficult condition, all cases can be completely cured. "Kaku" refers to food stagnating in the chest, not descending, and being vomited immediately. This condition cannot be cured, so do not use [the medicine] for it. Kaku occurs only in adults and is rare in children. Stomach reflux occurs in both adults and children.
○ Eye Diseases
The eyes are the essence of the five organs and the vital part of the whole body. Though there are said to be 72 types of eye diseases, they are likely due to spleen deficiency and weakening of the five organs, causing eye cloudiness and pain, night blindness, or cataracts, external and internal eye disorders, and various other manifestations. Since this medicine tonifies the fundamental spleen, not only cloudy vision but also night blindness can be cured in four to five doses.
