事を 上天（てん）に祈（いの）り奉る其／趣意（こゝろ）は抑（そも〳〵）此／脾肝薬王円（ひかんやくわうゑん）はわが
家方（かはう）にて家方（かはう）ならず所謂（いはゆる）天我（てんわれ）此奇方（このきはう）を授与（さつけ）し給ひ
一心正念（いつしんせうねん）に精製（せい〳〵）し普（あまね）く四方（よも）に及（およ）ほして天下（てんか）の病苦（ひやうく）を患（うれふ）る
ものを救（すく）ふへしと命令（めいれい）し給ふ事を感得（かんとく）せりよつて薬品（やくしゆ）を能々（よく〳〵）
調（しら）べ善（よき）を撰（えら）び悪（あし）きを除（のぞ）き調製（てうごう）に心（こゝろ）を尽（つく）し能書（のうがき）
包紙（つゝみかみ）等（とう）麤畧（そりやく）にせず将又（はたまた）薬（くすり）を求（もと）むる人の生質（うまれつき）病根（びやうこん）を
診察（わきまへ）し此薬（このくすり）の当（まさ）に応（わう）ずると不應（わうせざる）とを考（かんか）へ其／応（わう）すべきに
薬（くすり）を与（あた）へ応（はう）しかたきには断（ことわり）て与（あた）へず適（たま〳〵）応（わう）ずべしとおもひて
服用（ふくよう）せしむれども一廻（ひとまは）りにて功験（こうけん）なきには又／理（ことわり）て再（ふたゝび）与（あた）へ
ず尤（もつとも）其 奇験（きけん）あると雖（いへ）とも《振り仮名：財｜さい｜かね 》貧（とぼ）しく求（もと）めかぬる輩（ともがら）は無料（むれう）
にて与（あた）へ品物たに礼謝（れいしや）を請（うけ）すして全快（ぜんくはい）まて施（ほどこ）す是（これ）聊（いさゝか）

現代語訳
事を上天に祈り奉る。その趣意は、そもそもこの脾肝薬王円は我が家方にて家方ならず、いわゆる天が我にこの奇方を授与し給い、一心正念に精製し普く四方に及ぼして天下の病苦を患うものを救うべしと命令し給う事を感得せり。よって薬品をよくよく調べ善きを選び悪しきを除き、調製に心を尽くし、能書・包紙等粗略にせず、また薬を求むる人の生質・病根を診察し、この薬の当に応ずると応ぜざるとを考え、その応ずべきに薬を与え、応じがたきには断って与えず。たまたま応ずべしと思いて服用せしむれども一回りにて功験なきには、また理って再び与えず。もっともその奇験あると雖も、財貧しく求めかぬる輩は無料にて与え、品物だに礼謝を受けずして全快まで施す。これ聊か上天に報ゆる意なり。故に貧しき人は身を恥ずることなく必ず来たり給え。

英語訳
I pray to Heaven above for this matter. The purpose is as follows: this Spleen-Liver Medicine King Pills is not merely my family's formula, but rather Heaven has bestowed this miraculous prescription upon me, commanding that I should prepare it with single-minded devotion and spread it widely in all directions to save those throughout the realm who suffer from illness. Therefore, I carefully examine medicinal ingredients, selecting the good and removing the bad, devoting my heart to preparation, never being careless with written instructions or wrapping papers. I also examine the constitution and root causes of illness in those who seek medicine, considering whether this medicine will be effective or not. I give medicine to those for whom it should be effective, but refuse to give it to those for whom it would be ineffective. Even when I think it should be effective and have someone take it, if there are no beneficial results after one round of treatment, I again refuse to give it a second time. However, even though it has miraculous effects, I give it free of charge to those who are poor and cannot afford it, not even accepting gifts as thanks, and I provide treatment until complete recovery. This is my small way of repaying Heaven. Therefore, poor people should not be ashamed and must certainly come.
