乍恐以口上書奉願上候御事

奉蒙 仰難有仕合奉存相勤罷在候處
近年病身罷成難相勤奉存候依之

恐れながら口上書をもって願い上げます御事
一 私は不調法者でございますが、町代の御役儀を
仰せ付けられ、ありがたい仕合わせと存じ、相勤めておりましたところ
近年病身となり、勤めることが困難と存じます。これにより
町代の御役儀の御赦免をしていただけますよう
願い上げます。以上
右の趣旨について御慈悲をもって願い上げました通り
仰せ付けていただけましたなら、ありがたい仕合わせと存じます。以上
