市野瀬村勝五郎の息子仙太郎と申す者、当巳年十八歳、私の寄弟子にいたし、御水役を相勤め、鍛冶職をさせたく存じます。これにより願い上げます。
右の趣旨について御慈悲をもって願い上げました通りお仰せ付けいただけましたら、ありがたい仕合わせと存じます。以上
