右之趣以 御慈悲宗門抜呉候様被
仰付被下置候ハヽ難有仕合奉存候以上

右願書末書印形致同五日於御役所ニ伊沢岩右衛門殿御取次ニ而
差上置申候処同七日内済噯ニ仕候様御沙太ニ而御下可被成候
下モ町丁代市左衛門方江申渡置候
乍恐以口上書奉願上候御事
私義不調法者ニ御座候処数度丁代御役儀奉 仰
蒙難有長々相勤罷在候処年罷寄御役義難相勤
奉存候仍之御赦免被成下置候様奉願上候
右之趣以 御慈悲奉願上候通被為 仰付
被下置候ハヽ難有仕合奉存候以上

上記の趣旨について、御慈悲をもって宗門抜きをしてくださるよう仰せ付けていただけましたら、ありがたい仕合わせと存じます。以上

上記の願書に末書印形を押して同五日に御役所において伊沢岩右衛門殿の御取次で差し上げ置きましたところ、同七日に内済みにするよう御沙汰で御下しになってくださる下知を町丁代市左衛門方へ申し渡し置きました。
恐れながら口上書をもって願い上げ申し上げます
私は不調法者でございますが、数度町代の御役儀を仰せ蒙り、ありがたく長々と相勤めておりましたところ、年を重ね御役儀を勤めることが難しくなったと存じます。つきましては御赦免くださいますよう願い上げます。
上記の趣旨について、御慈悲をもって願い上げました通りに仰せ付けていただけましたら、ありがたい仕合わせと存じます。以上

The above petition was sealed and submitted on the same fifth day at the government office through the mediation of Mr. Izawa Iwauemon. On the same seventh day, an official directive was issued stating that the matter should be settled internally, and this instruction was conveyed to town representative Ichizaemon.

Although I am an incompetent person, I have been honored to receive appointments as town representative multiple times and have gratefully served for a long period. However, as I have advanced in age, I believe it has become difficult for me to fulfill the duties of this position. Therefore, I humbly request to be relieved of this duty.
