一中古以来集権現式は
　牛頭天王之類其外仏証を以
　神號に相称候神社不少候

一、中古以来、権現（ごんげん）の形式、牛頭天王（ごずてんのう）の類、その他、仏教の証（あかし）をもって神号（しんごう）と称している神社が少なくない。いずれも、その神社の由緒を委細に書き付けて、早々に申し出るべきこと。

　ただし、勅祭の神社、御宸翰（ごしんかん）・勅額（ちょくがく）等のある向きは、これまたお伺いを立てた上で、御沙汰（おさた）があるものとする。その余の社については、裁判（さいばん）・鎮台（ちんだい）・領主（りょうしゅ）・支配頭（しはいがしら）等へ申し出るべきこと。

一、仏像をもって神体（しんたい）としている神社は、今後、改めるべきこと。

Item: Since the medieval period, there has been no small number of shrines that use the style of "Gongen," the Gozu Tennō, and similar forms, or that employ Buddhist terminology to designate their divine titles (shingō). Each and every one of these shrines is to submit a detailed written account of its history and origins as soon as possible.

However, regarding shrines that receive Imperial ritual (chokusai), or those in possession of Imperial autograph documents (goshinkan) or imperially inscribed plaques (chokugaku), these too are to submit an inquiry \[to the central authorities\], after which an official ruling will be issued. For all remaining shrines, the matter is to be reported to the judicial authority (saiban), garrison commander (chindai), domain lord (ryōshu), or administrative overseer (shihaigashira), as appropriate.

Item: Shrines that have been using a Buddhist image (butsuzō) as their object of divine worship (shintai) are henceforth to rectify this practice.
