私義御水役相勤大工職仕罷在候処相止御鑑札
御上納仕外商売致居候処此度御水役相勤
大工職仕度奉存候依之御鑑札御下渡被下置候様

現代語訳
恐れながら口上書をもって願い上げ奉る事
私は水役を勤め、大工職を仕っておりましたが、やめて鑑札を上納し、他の商売をしておりました。しかし、この度水役を勤め、大工職を仕りたく存じます。つきましては鑑札をお渡しくださいますよう願い上げ奉ります。
右の趣旨について、ご慈悲をもって願い上げました通りお仰せ付けくださいますならば、ありがたい仕合いに存じ奉ります。

Respectfully submitted as an oral petition
I previously served in water management duties and worked as a carpenter, but I ceased these activities, returned my license, and engaged in other commercial activities. However, at this time I wish to serve in water management duties and work as a carpenter again. Therefore, I respectfully petition that you grant me a license.
