乍恐口上書奉願上候御事
西然寺御寺内宗門ニ而罷在候源蔵与申者
妻子共ニ私年来由緒有之実躰者ニ御座候ニ付
今度申談当寅御宗門より鉾持町人別ニ相加へ
申度奉願上候
右之趣何卒以 御慈悲願之通被 為仰付
被 下置候ハヽ難有仕合奉存候以上

恐れながら口上書をもって願い上げ申し上げる件について
西然寺のお寺の境内で宗門に属しております源蔵と申す者の妻子ともども、私は年来縁故のある実体のある者でございますので、今回相談いたしまして、当寅年の宗門より鉾持町の人別帳に加えていただきたく願い上げます。
右の趣旨について、何卒ご慈悲をもって願いの通りお聞き届けくださいますれば、ありがたい仕合わせと存じます。以上
