小林金右衛門
右の者は病身となりましたので、帯刀の御救免を願い出ました。やむを得ない事情につき、これを承知し届け出ます。帯刀を免除しますので、今後は町役等の諸事について、外並み（一般町人）と同様に心得るべきです。
右の趣旨を申し渡すべし 以上
