やむを得ず願い出る場合は、医師の容体書（診断書）を添えて願書を提出すること。万一医師が偽りの書面を提出した場合には、厳重に処置することになるので、この旨を医師たちへも漏れなく予め伝達すべきである。

今後僧侶は苗字を設けて、住職中の者は「某寺職氏名」と名乗ること。
ただし苗字を設ける場合は管轄庁へ届け出ること。
